Malreduction and avascular necrosis in subcapital fractures of the femur.
نویسنده
چکیده
Five hundred subcapital fractures of the femur have been subjected to close analysis, and follow-up has been continued for as long as possible during the past fifteen years (Table I). In the course of this review, the quality of reduction has clearly emerged as the factor most commonly affecting both the early and late results of treatment. Accurate apposition of the fragments is already known to be conducive to union, but it also seems that the fate of the femoral head is determined by the degree of reduction. It has been found that subcapital fractures uniting in good position escape the radiological changes attributed to avascular necrosis, whereas these changes are regularly seen when union occurs after malreduction. These findings throw some doubt on the belief that ischaemia is the cause of breakdown in the capital fragment, and suggest that a biomechanical factor is involved.

Compression screw fixation for displaced subcapital fracture of the femur. Success or failure?
We report a prospective study of 198 cases of subcapital fracture of the femur treated by closed reduction and fixation with a sliding compression screw-plate. This was done without regard to the patient's age or the Garden stage of the fracture. Early weight-bearing was encouraged. Of the displaced fractures 23% failed in the first year because of non-union or infection. Of the fractures which...
متن کاملClinical Results of Internal Fixation of Subcapital Femoral Neck Fractures.
BACKGROUND Subcapital femoral neck is known to cause many complications, such as avascular necrosis (AVN) of the femoral head or nonunion, compared with other femoral neck fractures. The purpose of this study was to analyze the incidence of AVN and fixation failures in patients treated with internal fixation using cannulated screws for the subcapital femoral neck fractures. METHODS This study...

We used technetium-99m antimony colloid to visualize the bone marrow of the head of the femur within twenty-four hours after interruption of the blood supply by subcapital osteotomy and section of the ligamentum teres in thirteen rabbits and within twenty-four hours after a subcapital fracture in thirty patients. Of the rabbits, all showed loss of marrow radioactivity over the affected femoral ...
متن کاملAvascular necrosis of femoral head presenting as pathological subcapital neck of femur fracture: An unclassified presentation
Avascular necrosis of femoral head is not an uncommon chronic disorder of hip in adults. The patient presents to the health facility with varied range of symptoms based on the degree of collapse of the head and arthritic changes at hip. Avascular necrosis of hip presenting as subacute condition with pathological sub capital neck of femur fracture has not yet been reported. The commonly used Fic...
متن کاملThe Correlation between the Fracture Types and the Complications after Internal Fixation of the Femoral Neck Fractures
PURPOSE This study aims to determine the correlation between the fracture patterns and the complications in patients with femoral neck fracture treated with internal fixation. MATERIALS AND METHODS The study comprises 45 patients with femoral neck fracture treated with multiple screws or compression hip screw between May 2008 and April 2012. The mean age was 48 years at the time of the surger...
